Monitoring vital signs such as weight, blood pressure, oxygen level, and heart rate are important tools we use to determine stressors that could be going on in your body. We also obtain laboratory studies to assist with adjusting meds. We monitor these over time to determine patterns and changes that may be occurring in your body.

Our hope is to prevent hospitalizations, however, in the case that hospitalization is necessary, we will follow up with you within 48 hours of discharge to help prevent a recurrence.

 Our goal is to develop an individual plan that focuses on decreasing the number of unnecessary medications you are taking, while making sure you are taking those that you need. In addition, we have found early intervention to be key in preventing hospital visits. Within your dedicated plan, we will educate you on warning symptoms that will alert you to the need to call us.

The disease process and progression can be difficult to navigate. We focus on educating you on what to expect as well as warning signs to look out for. We will also work with your specialists and family to ensure they are up to date on new concerns you may have.
