Concierge Home Visits
Our Medicare-covered private home visit service area is within 3 miles of QOALA's mailing address in Buford, GA. All eligible patients within this area may receive standard Medicare-covered home visits, billed separately to insurance providers.
QOALA Medical Services is pleased to offer an optional premium concierge service for individuals residing in independent living or private homes. This service extends travel beyond the standard 5-mile Medicare private home service area, up to 15 miles of Buford, GA, and provides additional support, coordination, and convenience that Medicare does not cover.
For an annual fee of $2,000 per client, we provide non-covered concierge services. Enrollment in the concierge program is optional and does not affect access to, timing of, or quality of Medicare or insurance-covered medical services delivered by our licensed Nurse Practitioner. Our home visit service is currently limited to a capacity of 20 patients annually.

Program Overview
This concierge program is designed to support aging in place, reduce avoidable hospitalizations,
and promote peace of mind for families and patients alike. Services will include both non-
medical concierge offerings and medical oversight by a Licensed Nurse Practitioner.
Services
Services Included Under Annual Concierge Fee ($2,000/year)
The following services are included in the annual membership and are not billable to insurance:
• Medication organization (non-clinical support)
• Appointment coordination and scheduling assistance
• Health education and preventive guidance
• Social needs assessments and referrals to community resources
• Priority scheduling and direct line access to NP care team
• Home safety assessments
• caregiver education
• assessment for need to transition to assisted living
Insurance-Billable Services (Provided by Nurse Practitioner)
The following medical services will be billed separately to the client’s insurance:
• Comprehensive medical evaluations
• Diagnosis and treatment of chronic and acute conditions
• Labs
• Chronic Care management services
• Medication management and prescribing
• Annual wellness visits and preventive screenings
• Advance Care Planning
Patients will be responsible for any copays/coinsurance.
Intended Audience and Financial Consideration
This concierge program is best suited for private-pay residents in moderate to higher-income
independent living communities/private residential homes who are seeking an elevated level of access, continuity, and personalized support beyond what is typically provided through standard insurance-based care.
Due to the fixed non-refundable annual membership fee of $2000, this service is not currently designed for residents in low-income or subsidized independent housing, where financial constraints may limit access to non-covered services. We remain respectful and supportive of all older adults and continue to advocate for models that serve diverse economic needs; however, this
particular offering is intended for individuals or families with the financial means to invest in
proactive, enhanced care coordination.
