Her Story

Nelum Walker, APN, FNP-C

​

Nelum began her medical career over 20 years ago and has worked as both a registered nurse and later a Family Nurse Practitioner in oncology (cancer), cardiovascular (heart) ICU, pediatric and adult primary care, pediatric hematology (blood disorders), infectious disease, and most recently chronic and palliative care. With each experience, she found a love for chronic and palliative care when she began visiting patients in the comfort of their own home. She quickly was able to see the whole person while getting to know them as more than just a patient in need of nursing support. Nelum's experience, clinical skills, attention to detail, and caring personality come highly recommended from her patients and their family members. Her passion is to help people live the best lives they possibly can while living with serious illnesses.

Nelum currently resides in Gwinnett County, GA with her husband, 4 children, and 2 dogs. She is an avid foodie-enjoying visiting new restaurants, date nights, and loves spending time with her family and friends. Traveling, enjoying the summer season, watching her kids pursue their own dreams, and being a volunteer leader to her high school students are some of her favorite things.

Maya Trocheck, APN, FNP-C

Maya began her medical career 11 years ago and has worked both as an RN and Family Nurse Practitioner in oncology, orthopedic/trauma, medical/surgical unit, progressive care unit, cardiology, and hospice/palliative care. Her experience with hospice/palliative care taught her the importance of treating patients as individuals (spending the extra time to make sure they feel heard and seen). She strives to help her patients live their best lives.

Maya currently resides in Hall County, GA with her husband, 3 children, and dog. She enjoys traveling, meeting new people abroad, hiking, being in the water, and spending time with her family. She has been on mission trips to Nicaragua, and spent a month in Ghana- the trip that inspired her to become an NP to make a difference.
